Do I have a yearly maximum? If so, do I have a separate maximum of major (crowns/bridges/partials) and for basic (fillings/extractions) or do I have a combines maximum for both?

What percentage is covered? What year does your insurance base their fees on? (We always base our fees on the current year Ontario Dental Association fee guide. Your plan may be using fees from a different year).

Does my coverage run from January 01 to December 31 (calendar year) or does it run from another date?

Are my reacll exams (code 01202) covered by my plan every 6m/9m/12mnths? Recall exams may be covered more often for children than adults.

How many units of scaling (code 11114) does your insurance cover and is it per calendar year or per 12 rolling months? (1 unit of time= 15 minutes with the hygienist). Your plan may cover 6 units per year but may cover additional units if we submit an estimate on your behalf for more scaling.
